Passport
Applicants must fill out a registration form which includes full name, current address, phone number and email address.
Borrower Responsibility:
A valid library card must be presented when checking out library materials. It is the patron's responsibility to keep their borrowing privileges in good standing. A patron with outstanding material or an overdue fine of $5.00 or above, will have their borrowing privileges suspended. Borrowing privileges will be reinstated after the patron has returned any outstanding material in its original condition, paid for damaged materials, or has brought their overdue fine below $5.00 and continually pays on the balance.
Library Cards are issued for the sole use of the registered borrower and are not transferable. The library card holder is responsible for all materials borrowed on his/her library card. A parent or guardian must come into the library with identification and sign the registration form for a library card for a minor under the age of 18. The parent or guardian is responsible for all materials borrowed on the minor's card. The parent or guardian and minor will have their borrowing privileges suspended if the family's cards have a total of $10.00 or more in fines and fees. A card holder 18 or older but not the head of household is responsible for his/her patron record.
Library cards are like credit cards and allow their possessor to borrow hundreds of dollars worth of materials. The library assumes that the person using the library card is the owner of that card or has the permission of the owner to use the card.
The library card holder is responsible for informing the library in the event of a lost or stolen card. All materials checked out on a library card are the responsibility of the patron until the loss is reported. Until the library is notified of a lost or stolen card, a library card is valid and its owner is responsible for all use of the card and for any lost or overdue materials and fees incurred..
Library cards lost are damaged beyond use may be replaced with a payment of $5.00.
The borrower is responsible for informing the library in the event of changes in name, address and/or phone number. Change of address should be reported promptly, the library may not be able to contact you about important information if your address is not up to date.
